MANDATORY PHYSICAL EXAM & IMMUNIZATION
Pre-registration WILL NOT be complete until all Tomorrows Stars Campers submit a copy of their most recent physical exam and immunization records to our office with your application, or as soon as possible thereafter. In order to participate at the camp, this physical must have taken place within the last two years. Due to the Board of Health regulations, each camper must submit an updated copy of most recent physical & immunization record regardless of previous attendance at camp. Campers may utilize the following Tomorrow’s Stars Basketball Clinic Physical Form, or a comparable form received from their physician's office. Physical/ Immunization Form

Tomorrows Stars Basketball Clinic complies with regulations of Massachusetts Boards of Health, and is licensed by the local Boards of Health.

You will be required to complete and sign a medical release form prior to attending any sessions. Forms are available by mail request or clicking this printable application , medical release and release and consent form
